When I began my public health career, I was motivated by a clear sense of purpose. I wanted to prevent disease. I believed that if you cared enough and worked hard enough, you could make a meaningful difference. Passion felt like the essential ingredient.
I was not wrong that passion matters. But I was wrong about what it can accomplish on its own.
Passion without systems thinking produces well-intentioned programmes that fail at scale. Passion without political literacy produces technically excellent interventions that cannot get funded or institutionalised. Passion without data discipline produces advocacy based on anecdote that cannot survive scrutiny. Passion without cultural humility produces interventions that communities reject because they were not designed with or for them.
The moment I began to understand this was during my first field assignment. I was working on a disease surveillance programme that was, on paper, well designed. The data flows were mapped. The training had been delivered. The forms were printed. And then we discovered that health workers at the facility level were not submitting reports because they had no incentive to do so, the process added to an already overwhelming workload, and they had seen previous surveillance data collected and never acted upon.
The passion was there. The health workers cared about their communities. The programme designers cared about outcomes. But passion had not solved the incentive problem, or the workload problem, or the trust problem. Those required different tools: systems analysis, stakeholder engagement, process redesign, sustained follow-through.
What public health requires is not less passion, but passion disciplined by rigour. The urgency that passion provides has to be channelled through careful analysis, evidence-based design, institutional understanding, and the patience to work within systems while also trying to change them.
Nine years in, I still feel the urgency. But I am more systematic about how I respond to it. That shift has made me more effective, not less.
